Land Registry Form Ap1

Hello

How do I change the address regsitered with the Council and the land register? Basically, I want to take the estate as we were on the list and the name of the house, the name or the road, the village name, the name of the county and zip code.

Does anyone know how to do this, the costs involved and advice would be greatly appreciated? I have as far as finding a form AP1 but not sure whether or not this is fair? THANKS!
